As an RN Clinical Coordinator, you’ll play a vital role in bringing that mission to life. You provide nursing assessment, care coordination, education, and ongoing clinical management for an assigned panel of PACE participants.

### Responsibilities

- Coordinates nursing care and follow-up for an assigned panel of PACE participants.
- Completes required nursing assessments.
- Reviews relevant clinical information, diagnostic results, and outside records using nursing judgment and critical thinking.
- Develops, reviews, and updates individualized plans of care.
- Participates in interdisciplinary care planning and clinical discussions.
- Coordinates and follows up on provider orders, referrals, diagnostic testing, specialist recommendations, medication changes, and other identified care needs.
- Supports transitions of care following hospitalization, emergency department visits, skilled nursing stays, and other changes in care setting.
- Provides participant and caregiver education related to disease management, medications, treatment plans, symptom monitoring, and preventive care.
- Triage and provide appropriate nursing intervention.
- Maintains complete, timely, and accurate documentation of nursing care and coordination activities.

### Skills

- Graduate of an accredited School of Nursing.
- Must have three (3) years' experience as a practicing nurse in a health-related area.
- Licensed Registered Nurse in the State of Michigan.
- Must possess a valid driver's license.
- Must be CPR certified.
- Minimum of one (1) year working with the frail and elderly.
