### Job Summary

The Clinical Chart Abstractor – Pulmonary Rehabilitation is responsible for reviewing patient electronic medical records (EMRs) to determine medical eligibility or ineligibility for Aviary Health’s virtual pulmonary rehabilitation program.

This role supports the enrollment team by reviewing medical histories, identifying qualifying pulmonary diagnoses, interpreting relevant clinical documentation, and verifying that referral requirements are met. The Clinical Chart Abstractor will document eligibility determinations, identify missing information, and coordinate follow-up to support timely patient enrollment.

### Responsibilities

- Provide daily chart review support to the enrollment team by evaluating patient medical records for pulmonary rehabilitation eligibility and ineligibility.
- Review pulmonary diagnoses, relevant medical history, pulmonary function test (PFT) results, and other clinical documentation to determine whether patients meet established program eligibility criteria.
- Identify qualifying pulmonary conditions, including COPD, emphysema, interstitial lung disease (ILD), and other eligible respiratory conditions, as defined by program guidelines.
- Review ICD-10 codes and supporting clinical documentation to confirm eligibility according to established inclusion and exclusion criteria.
- Identify existing pulmonary rehabilitation referral orders and coordinate the creation or completion of referral documentation when needed.
- Verify referral requirements, including appropriate ordering provider credentials, signatures, and cosignatures when applicable.
- Identify missing, incomplete, or conflicting clinical documentation and coordinate follow-up with the appropriate team members.
- Communicate eligibility determinations, ineligibility reasons, and outstanding documentation requirements to the enrollment team through concise, accurate documentation in designated platforms.
- Maintain organized, accurate records of chart reviews and eligibility decisions within the EMR and other designated systems.
- Escalate cases that fall outside established eligibility guidelines or require additional clinical review to the appropriate clinician or program leadership.
- Assist with special projects related to pulmonary rehabilitation enrollment, chart abstraction, and clinical operations as needed.
- This position requires strong attention to detail, the ability to interpret clinical documentation, and the ability to make timely, consistent eligibility determinations using established guidelines and decision trees.
### Qualifications

- 3–5 years of clinical experience in pulmonary care, respiratory therapy, exercise physiology, cardiac or pulmonary rehabilitation, or chronic disease management.
- Experience reviewing electronic medical records and interpreting clinical documentation.
- Familiarity with pulmonary conditions, particularly COPD, emphysema, and other chronic respiratory diseases.
- Working knowledge of pulmonary function tests, spirometry, and relevant diagnostic terminology.
- Ability to interpret ICD-10 codes and apply established clinical eligibility criteria.
- At least 3 years of experience with EPIC or another electronic medical record system.
- At least 2 years of experience with medical terminology and clinical chart review.
- Proficient computer skills, including experience with Google Workspace, Zoom, and electronic medical record systems.
- Strong written communication skills, with the ability to summarize clinical findings and eligibility decisions clearly and accurately.
- Excellent organizational skills, attention to detail, and the ability to manage multiple tasks independently.
- Ability to recognize cases requiring clarification or escalation rather than making assumptions when documentation is incomplete.
